Ok, I ended up hacking the server source to turn off the cursor.  Turns out
that the server assumes an IDC_ARROW cursor on initialization, and captures
the current cursor when the mouse is moved.  I'm still not sure why setting
the cursor to a blank bitmap in the control panel didn't turn it off.  Might
have been the IDC_ARROW default.

The changes I made to turn off the remote cursor were in vncDesktop*.cpp,
vncMenuc.cpp, and VNCHooks/VNCHooks.cpp.

If somebody wants a "diff -u" patch, let me know.  It ain't pretty, but it
works for me. :-)

That turns off the local cursor.  I want to zap the remote one and keep the
local dot.

I did try changing the cursor in the control panel to a blank graphic, but
that didn't seem to work.  Is the server sending the default arrow even when
none exists?
