Try removing a line that looks like this (with different numbers) from
your .vtclrc file (you can find it in your home directory). This should
reset the toolbar to its default position.

set vTcl(geometry,.vTcl.toolbar) 66x572+0+194

What OS are you running ? Some Unix Windows Managers have the really bad
habit to not place windows where you want to them to be, shifting them
by 10 or 20 pixels everytime.
