I don't understand what this has to do with vhosts.
Either the contacted web server acts as a open proxy or passes the request to 
the extranet host.
In both cases you don't know which one sends the response you see, could be the
contacted server, could be the target server or could be extranet server.

Only if you can ensure that extranet answers, then it's most likely a vhost on
the connected server.
