>From memory you can set a preference to have it show every time, but
there are two things that make me ask Why?

1. If the extensions that you wish to switch off are already loaded by
the time the extensions manager comes up, you can turn them off and hit
CMD-R to restart the machine with the current set.

2. It sounds like you wish to choose a set of extensions every time you
start up because something in the Macintosh environment changes -
devices hooked up, needs, location, etc. If you use the location
manager, then you get (if you choose to) a dialog box with a pop-up that
allows you to choose your settings, setting preferences such as
extension set, printer, location, time-zone, sound, etc., all with one
(actually two - you need to click ok :-) click.
