Try isolating ~/library/caches/com.apple.help.ui (or wherever it's
located in 10.4.x)
Getting rid of those caches works in 10.3.9

This problem may have been fixed in 10.4.6, released yesterday and
available from Software Update. Certainly, the problem where MacHelp
occasionally displays a blank window has been addressed. Hopefully,
this might have been part of the same problem.
