I have just returned from time in Italy - ship and land.
Most ships are incredibly unreliable, expensive and very slow with very high 
charges for internet/wi-fi connection - for either your or ship computers. (I 
have experienced up to half an hour to send 1 email and you pay by the 
minute!). The last ship I was on did not support any Apple products - only 
their MS computers.
It often did not feel safe walking around some of the towns carrying/using my 
iPad but there were often internet cafes (very cheap) or internet/wi-fi 
connections available at ports or close by.
Many hotels offer free internet connection using their machines. Internet/wi-fi 
connection in your room is more expensive.

I didn't use my phone much but heard good reports of buying a local sim card.
