Usually I added 0000-00-00: Version 2.1 <next unreleased version> for the 2.1 branch, and similar for the trunk (though if you are going to do it for trunk, be aware that everyone was lazy and nothing yet was recorded, it will be a lot of work, and I would just include the most major changes if I were you).
I tried to sort the changes into topics, and ordered them from New (feature), over Change (in behaviour) to Fix (of a bug). Short but meaningful sentences, and if possible add a bug or patch number to the end of the line. (Sometimes needed to search/guess and/or ask the patch author when I had the feeling there was a bug associated but not mentioned in the svn-log.) And only add user visible changes of a sensible size. I think that should be all. Thanks! Dennis
