Fix cheats "double up" and "biffer baker"
* remove difficulty levels DL_TOUGH and DL_KILLER
* fix cheats causing undefined behavior in campaign
* fix loading and saving of difficulty levels
* re-implement cheats so that they change nothing but damage modifiers
