When I was growing up the term "lunch counter" indicated an eatery inside a larger store. The one I remember clearly was Woolworth's lunch counter. They would serve all sorts of things from sandwiches to hot lunches.
Some drug stores would also serve food from the soda fountain, but it was limited fare such as sandwiches. We didn't refer to drug stores as having "lunch counters."
