Yes, it does make a difference, and no, price isn't a deciding factor. If you register with an ICANN registered registrar, you will have faster response times in every area of domain management, including changing DNS settings, changing administration, billing and technical contact info, etc.. If you register with a second level or below registrar, you will have to wait for them to send the changes up the line so to speak. And I second the warning to be careful of any registrar who would register your domain in their own name - this is particularly relevant if you're registering through a web host.
I use GoDaddy.com. They are an ICANN registered registrar, and charge $8.95 per year. You can administer your domains directly online, and the changes are fast. No, I don't get anything out of recommending them other than wanting to support a company that I've been happy with.
