MIVA uses tags like html and cfml. My experience with it is confined to a shopping cart included in the hosting package but I have made some changes to the html in the cart so I've worked my way through the miva script.
With that thin bit of experience in mind, what I'd do is find the relevant parts of the miva script and check the names of the variables. If the choice page simply passes along the same vars, I'd try editing out the choice so the script went from selection directly to shipping and billing. If the vars change names, there is a miva script book. Its a lot like trying to learn cfml when you know html -- familiar but slightly odd. There are also a number of miva sites with helpful hints and tutorials. They can select to purchase without registering, and it takes them to a information page for shipping and billing information, and past that the order is confirmed, and is captured in miva for me to process.
