If you're testing on a remote server instead of your local machine, make sure the filename of the image file is EXACTLY the same as in the HTML code, i.e., make sure the case matches. I've had a problem before where the HTML called for image.jpg and then the image file was edited and saved as Image.jpg. Worked fine locally (Windows) but not when uploaded to the server (Linux). Might not be what the problem is, of course, but worth looking at.
