And just how can they bill someone for a name before there was a fee?

I have several domain names that I registered way back before there was a
fee (incl. my own www.andreas.com). I pay the current fee, but under
protest, since they can't simply create fees on their own. There's a lawsuit
going on about this and maybe someday, I'll get my money back.

I've never heard of them cancelling an account.

What they say and what they do is two different things. They want people to
pay, but many simply don't. How else can one explain a Canadian company
sitting on 10,000 domain names? The fees alone would be $350,000/yr., but I
seriously doubt they are paying. It'd be far more profitable to invest in
the stock market.
