A few months ago I asked for a critique on a system I was developing 
to build an HTML form and the Perl script to process the form's output. 
Quite a few people made suggestions and I returned to the drawing board. 
The new version is ready for review if you have some time for a critique.

     The new version has the following features.

1. Supports the form elements: text, radio buttons, text-area, and check 
   boxes.
2. If required fields are not submitted, errors messages are produced.
3. Optionally, a nicely formatted e-mail message containing the submitted 
   information is sent to a user-supplied address.
4. Optionally, the submitted fields are appended to a user-supplied file 
   name and field delimiter..
5. User may select either CGI.pm or cgi-lib.pl to parse inputs.
6. Commonly used input fields are selected by just checking.

     As before, your input would be appreciated. A short demo is

There is a link on the demo page which takes you to the system's first 
input page. Both the finished HTML document and the Perl script to 
process the form will be e-mailed directly to you.
