I had this problem and fixed because there was no "/tmp/mysql.sock" ...now your mileage may vary but make sure that your webserver can connect to the db. Find out what your default connection is by running a phpinfo file. The contents look like this: # <? phpinfo(); ?> # #

So, I open a browser and load the web-cyradm page. I enter in "admin" and my password and the next screen is plain white with the error "Database Error", nothing else. This occurs even if i log in with a non-existent account, so it seems that I am not being authenticated. MySQL is running and PEAR is installed. I have set the include path as well.
