I looked over the code, and I think it is possible to be made.
Because if you create user "user" and user "user.domain.com" they can be
seen as 2 distinct users.
Of corse for 2 distinct domains with free names you cannot create 2 users
with the same name. That is obvios.
I had no time to look a little more about this thing (for me it was only a
observation not a priority) but if I make somthing I will anounce u.

I'm also using "free names" option (after editing the source), and I'm aware
of this limitation - do not create same usernames for different domains.

The problem that I noticed is that the usernames are made only with
$DOMAIN_AS_PREFIX condition and the domain "free name" check is not
took into consideration.
DOMAIN_AS_PREFIX (or unixhierarchysep) was developed from cyrus. When you
edit the source code you break the idea from cyrus and not from web-cyradm.
It's like you edit the POP3 protocol and abuse Microsoft that Outlook
Express doesn't pop mails anymore ..
That's why it doesn't work ...
