If you are using the standard Web-cyradmin, try also the CVS.
Once I had also a problem with login(this was software related but don't
remember what was the exact cause of it).
Are you sure the all the processes are running and that are configured
correctly? Mysql, cyrus, sasl?
I don't know if a right access to the log file would cause so much problems.
Maybe a warning. But not a script Die.

what is your www user? your file as 2 users which can write to it.
Ensure that the file can be written by the user apache or whatever your web
server runs as.
