It looks like the problemw was with building pam_mysql the first time around.  After rebuilding it using a different md5.h and re-installing I was able to get things to start working properly.

Turn on logging in mysql server (i.e. /var/log/mysql/log) and examine
all sql queries done by postfix and pam.

In you correctly add user in web-cyradm then you should have proper
files in your /var/lib/imap and /var/lib/spool directories.

Can you see persentage usage of quota for this account in web-cyradm?
