Now that you mentioned, I got "(unknown)" in the host column also (just turned sqllog on). I think pam_mysql is working right, since there's no errors in syslog, and the column gets populated. I think problem is lying in saslauthd not being able to pass the host to PAM. I tried sqllog on both cyrus and postfix via saslauthd (same results), and I'm about to test pam_mysql via some regular service (thinking ftp) to see if it works. I report back as soon as possible.
