I have had success with just fieldnames on the first row. Also, in earlier versions of the web2py (1.5 or earlier) there were invalid ^Ms in the file. I fixed these (but don't have to now in current version) by editing in vi and doing :%s^V^M/^V^M/g (where ^ is pressing the ctrl key)
