perhaps you can create an address table and use list:reference in 
sqlform.factory
e.g. (not tested)
db.define_table('address', Field('address') )

def form_from_factory():
    form = SQLFORM.factory(
        Field('address', *'list:reference', requires = IS_IN_DB(db, 
db.address.id,  '%(address)s')* ) )
    if form.process().accepted:
        response.flash = 'form accepted'
        session.address = form.vars.address
    elif form.errors:
        response.flash = 'form has errors'
    return dict(form = form)
