I'm confused by the use of .process() at the end of your SQLFORM 
instantiation.

Instead, I would do this:

form = SQLFORM(db.certificate_request)

if form.process().accepted:
    response.flash = 'new CSR registered'
    redirect(URL('customerArea', 'index')
