I tried:
gender=SQLField('gender',requires=IS_IN_SET(['Mr.','Mrs.']))
gender.widget=radioboxes
form=form_factory(SQLField('...'),gender,SQLField('...'))
When I expose the function an error ticket is issued:

I guess I need an import statement ...
