Clean-up Bugzilla login dialog

Reviewed by Alexey Proskuryakov.
Sets the "Remember my login" to baseline alignment and
changes the login dialog to a modal, autofocusing the email
input field with refined sizing to accommodate more varied
mobile viewports.
