Make sure provisionalOrCurrentIndex stays in bounds in
WebBackForwardList::addItem

When removing an entry, if there are no more entries, then we have no next or
provisional
index. This will prevent a crash where we try to read out of bounds but
Vector::operator[]
crashes for us.
