Move AnnexB utilities to a specific file

Reviewed by Jean-Yves Avenard.
Given AnnexB is used by H264 and HEVC, we move the utilities to a new file
AnnexBUtilities.h, which is used by H264/HEVC utilities.
We directly use a Vector<NaluIndex> to cope with the fact that H264 does not a
vps.
We update the code accordingly.
No change of behavior.
