RemoteInspector::setup: Ignore Setup for a target that already has a
connection

The Cocoa implementation (RemoteInspector::receivedSetupMessage in
cocoa/RemoteInspectorCocoa.mm) ignores a Setup message for a target that
already has a connection. Do the same in the GLib and Socket implementations
instead of replacing the existing connection.
