Fix code blocks in WebKit Code Style Guidelines disappearing shortly after
loading

Reviewed by Timothy Hatcher.
Adds an improvement to set the heading level to generate a table of contents
so one properly shows up for the code styles (which use H3 all below the
original
threshold). Now the threshold can be set arbitrarily in a post/page using
metadata.
Removed all complex CSS styles causing layout issues with code blocks beside the
floating table of contents.
