That may be your problem. It is very sensitive to reverse lookup
problems.
There is nothing in the adaptor log for server b, we are under the
impression that it would only show a log for the machine that is
accepting the apache requests.
I didn't mean the log on server B (there won't be one) but the domain
name or IP for server B that appears in server A's log. Is it
possible that the apps on server B are listening on a different IP /
name?

As you can see instance 6 is working great, but when we disable
this instance or refuse new sessions on it WO is unable to use the
server b instance 9 app and you receive a page not found.
