Deselecting the Performance Cache option in OS X server seems to have rectified the problem. If it is selected (which seems to be the default in 10.4), it creates a virtual host that  listens on port 16080 instead of port 80. I'm not sure how this effects the delivery of the webserver resources, but I'm no longer seeing the 501 error and the images are not hanging.
