I've seen this as well. It occurred when I use the browser 'back' button. But I was also able to replicate the problem when in a form, I download a file (which doesn't refresh the page, the response get written to disk), after that, when the form get submitted, all values are null. In our case, after investigation, we didn't want 'browser back button' nor a file download inside this page.
It can be a hint for someone looking for a real fix.

Just a strange idea: do you do something in awake() which changes the values for the conditional? If you set a boolean to false, and this effects a conditional wrapping a text field, the field "disappears" from the scope of the form as far as I know. Chuck described this with a hyperlink being clicked but the method is never called in his book.
