3) Why are you doing an onClick with an Ajax.Updater in it vs just having updateContainerID = "content"? That looks particularly suspicious to me.

because updateContainerID in AjaxSubmitButton has never worked! I've found this solution on the AjaxExample and it works!
I use updateContainerID all the time on AjaxSubmitButton, so it definitely works. This makes me think you might not be up to date.
In the case that AjaxSubmitButton did not work for you, did you have an AjaxUpdateContainer INSIDE of a form rather than AROUND a form? I just tracked down (and fixed) a bug in this scenario.
