ok so the error_log shows: File does not exist: /var/www/html/WebObjects/JavaMonitor.woa/Contents/WebServerResources/help.png
and indeed the file is not there they live here: /opt/Local/Library/WebObjects/JavaApplications/JavaMonitor.woa/Contents/WebServerResources I may reinstall Centos6 and do the WO installer again as this box was originally setup before Pascal created his beautiful YUM installer.
