I am interested in how others are handling these disclosure lists.  We are a medium size hospital and physician clinic.  We have a great deal of our medical record computerized but not all of it.  But there, apparently, is much more to the disclosure list than just the medical record.  In previewing a HIPAA film on staff education, it was presented that all those who view the protected health information need to be listed on a “disclosure” list so that if the patient asked they could find out who all viewed their records.  I have been preparing to have a disclosure list for every place I sent the information outside of the facility, but I was certainly not thinking of having a list of everyone who viewed the information during the course of a patient’s care.  What are others doing in this regard?
