The NM108 and NM109 data elements are more correctly (under the X12 syntax)
termed paired data elements. Here's the actual syntax rule from the X12
standards, which of course applies to all uses of the NM1 segment in
whatever HIPAA transaction set it's used:

Syntax: 1. P0809
If either NM108 or NM109 is present, then the other is required.

I believe NM108 and NM109 are co-requisites for most loops.  Wanted to throw
that out there just in case.
