John already answered that it could be "internal testing" and if CMS allows somebody 
to start as late as 04/03 to start their "internal testing", and if many do, I am 
afraid that can be one of the grounds for another extension!

DAs the ASCA extension application was being drafted, several of us tried 
very hard, in face to face meetings, to pressure CMS to have CMS define 
testing as external testing.  Obviously to no avail.
