You sould probably be looking at the 837 p - this transaction does include
information for ambulance billing.

I have my doubts about a direct mapping, but there may be a mapping from
the Medicaid edition of the HCFA 1500 paper form (in our state, the
Medicaid paper form [ILDPA 2360 02/1990] is vaguely similar to the
standard form -- they eliminate some things, move others, and add some
unique to their purposes) to the National Standard Format. Check with your
state's Medicaid and with CMS (they definately have a mapping from the
standard HCFA 1500) The Implimention Guide for the 837 then contains a
mapping from the NSF to the 837 (Appendix F).

Greetings,
I'm a new kid on the block and am trying to find the shortest route to
producing an 837 for our county transportation department. We currently
build a batch file based upon the HCFA-1500 Format Version 3.1. Our only
concern is Medicaid billing for the transporting of Medicaid clients to
and from Medicaid approved destinations.
Does anyone have any suggestions as to where I can find mappings from my
old format to segments in the 837 or suggestions as to where to start
looking?
