The "more stringent" provisions are part of privacy, so I assume you are not talking 
about transactions. In any event, if you are trying to decide if you are a covered 
entity under hipaa, start with this tool from the CMS website:
 http://cms.hhs.gov/hipaa/hipaa2/support/tools/decisionsupport/default.asp

For public health, I suggest you start with "Are you a covered health care provider?"

If you are, in fact, a covered entity your next step is to determine how your uses of 
protected health information are defined, and how the privacy rule applies to each 
use. There is a lot of help for that available on various sites. You might start your 
search at
