The guidance from X12 on this topic is that you use: - P for the primary payer - S for the secondary payer - T for ALL subsequent payers, including also the payer of "last resort" such as Medicaid.
So the sequence P S T T T would be the one to use instead of P S S S S or even P S S S T as you suggest.
