A possible diagnosis from the IRC logs:
13:42 <zookeeper> my theory for Mythological (or whoever else might want to
look at it) on what goes wrong: the chamber for that leader (line 432) has
x=1-10 and size=5, which can cause the chamber to be partly off the map
(since i think size works like radius, and x,y is just the centerpoint). in
this case, the map is 50 wide and the leader position was 51,50, so the map
was probably flipped horizontally (flipx_chance=50, line 261)
