Follow-up Comment #2:

"Restart" there refers to restarting the current game (i.e. scenario) through
any mechanism, not restarting Wesnoth. I couldn't fit a longer explanation
without making the text unreadable on common resolutions (it already causes
the game to select a smaller font size on 800x600).

After further testing, it turns out the restart requirement is only valid
when the Help window hasn't been displayed during the current game, which
causes the unit tree to be forcibly cached. If you change the setting before
bringing up Help for the first time, it should work.

I don't feel particularly inclined towards removing this requirement at the
moment, or changing the tooltip's description during a string freeze, so I'm
marking this as Won't Fix.
