Details:

If you use [modify_unit] and specify a location the unit is moved to, this
causes another instance of that unit to spawn at the new location; however, at
the old location a unit with the same id remains.

I was able to reproduce the bug in both 1.10 and 1.11.

Attached is a testcase, simply copy it to a new folder and start the campaign
"[modify_unit] test".
