Dear Contemporaries,

 

I agree with the viewpoints of Milton - obtaining statistically defendable 
overall causes/triggers of these frequent accidents, voicing our concern, and 
then putting forth suggestive approaches to decrease on this frequency.

 

While establishing the cause of these accidents, given the skills that we have, 
let us try to talk to the machine operators themselves (I mean drivers,  their 
assistants - oka Turn Boys, and also the mechanics).  Thorough and confidential 
investigative  approach towards this particular segment of the system should 
yield for good data.
