If you want the device to be marked as Down when a service goes down, then set the device to be monitored using Services Only. (device Properties \ General)
 
When the alert is sent it will look something like:

NT Workstation mydevice.com Telnet  DOWN at 15:00:13

Any idea why a monitored service alert message reports a default “active and responding” when it should be the “down” message?

Specifically, I manually added Telnet service to a node (workstation) and stopped it to check alerts which reports active and responding when down.
