We ran into this problem, using a browser with multiple screens (NetCaptor). We reload the application every night shortly after midnight (in order to put a time/date stamp on anything that's down at midnight). Our operations staff would then reload NetCaptor, initiating queries for multiple maps simultaneously.   Since the CGI images are named using system time in the naming algorithm, sometimes the CGI names were duplicated because the browser was sending essentially simultaneous requests for different maps.
