If you are talking exchange, you could use the NT monitor to check that the service is actually running. But this will only report if the service stops (crashes). For the SMTP service monitor, try changing the number of events required to trigger an alert. Certainly we get notified when our SMTP service stops responding to whatsup because its busy.
