I was also thinking along the lines of IE's image toolbar. However, this is up to browser vendors to decide.

To ensure that video is always usable:
* UI would be on by default (but visible on hover)
* Author could disable it using attribute of <video> element
* User could re-enable controls using context-menu or some other method (in case author disabled default UI and failed to provide usable alternative)

Better if it's disabled with CSS -- that way it could be re-enabled by a user stylesheet, a mechanism already implemented in browsers.
