Summary: Missing copyright headers
Initial Comment:
AbortAndRespondException and friends, and probably a
lot of other classes recently added are missing the
Apache header we use on all source files. In addition,
there are some ugly looking blank lines and comments in
these files such as below. This is easily fixed by
using the eclipse code format for the project and
control-shift-f.
