The ad will essentially act the way that an image or an infobox does, moving
things on the right down a step. That will affect page design, and
communities will have to look at the new format and decide how they want to
adjust. Some wikis may decide that they want to change the way they approach
certain templates or infoboxes.

Luckily, these are wikis, so everybody gets to help with whatever
adjustments need to be made.

Personally, as someone who works on wikis all the time, it took a while for
me to get used to the new format. Initially, I had the same kind of gut
response -- I liked the way things were, and I really didn't want them to
change. Now that I've been working with the new format for a while, it's not
really as much of a change as I thought. It's still just one ad, but it's in
the right corner rather than spread across the top.

The first part of the change is going to happen on Tuesday -- the edit bar
will move up, and the ads will start appearing on article pages. Personally,
I'm looking forward to seeing how wikis adapt to the new format. Wiki people
are great at figuring out design tricks -- and when one wiki comes up with a
good idea, it can spread to others very quickly. It'll be interesting to see
what happens.
