When there are more pages than can show on a page of Special:PrefixIndex the
link "Next page (foo)" shows in the upper right hand corner as can be seen
here:
http://www.mediawiki.org/w/index.php?title=Special%3APrefixIndex&prefix=&namespace=0
However, when no starting page has been entered, the name of the immediately
succeeding page is treated as both the &from and the &prefix, frequently
resulting in a single entry rather than all of the succeeding pages as
expected. This can be seen by clicking the "Next page" link on the page above.
When a prefix is given, this does not occur as &prefix is fixed at that value
as one pages through the "Next page" links, as can be seen, for example, here:

It should be possible to use a null prefix and page through all of the pages in
a namespace.
